As we mentioned in a recent post; in the interest of serving our patients better and running everything more smoothly, we're making some changes here at Caldwell Pediatrics and Wellness Center. The initial changes will slow the check-in process as we update your child's information for our new system. We apologize for the wait, but it is necessary to ensure we have thorough, up-to-date information. This is a temporary issue, as we become more familiar with the system and gradually have everyone updated.

To speed the process along, we have forms available under the "SERVICES" tab here on the website. You can print and fill out your forms before you come in. Soon you will be able to fill your forms out digitally, as we continue our system upgrade.
